We look forward to working with you towards obtaining your healthcare goals. For your convenience, most of the paperwork you will need at your first appointment is available right here on our website. Please complete and sign the required forms and bring them with you to your appointment. The following forms are included in our new patient packet and are required prior to your appointment.

If you have additional forms or paperwork that are needed (school forms, employment forms, disability forms, etc.) we ask that you complete the patient portion prior to your appointment and bring all of the necessary paperwork with you.

Scheduling your first appointment:

If you feel that you need to be seen on an “urgent” basis, please contact our office at 509-332-3548. We can generally get you in for an appointment on the day that you call or at least within 24 hours, excluding weekends and holidays.

If you are in need of a routine or non-urgent issue you can request an appointment by calling (509) 332-3548. In the event of an emergency, you need to call 9-1-1.

Please note: You will need to provide your name, date of birth, insurance information or form of payment, current address, current phone numbers, and a brief description of the reason for the visit when requesting an appointment.

Arriving at your first appointment:

We ask that you arrive at least 15 minutes prior to your first appointment. This allows time for the registration process.

When you come to your appointment, you will need to bring your insurance information, a government issued identification card, and a form of payment for any applicable co-pays, deductibles, and/or services not covered by insurance.
